Sports Physical Information
 
Spring 2012 Sports Physical Dates:

WMHS - 2/28/12 @ 2:00 p.m.
Gelinas - 2/29/12 @ 2:30 p.m.
Murphy - 3/1/12 @ 2:30 p.m.

All athletes must be screened by their school nurse prior to the physical date.  The 3 forms necessary are located to the left "Athletic Information".

REMINDER
For participation on a spring sport 2011/12
all Private Physician Physicals -

JV & Var (Grades 9-12) DUE BY 2/27/12

7/8th grade DUE BY 3/12/12

Return all forms to the health office in your child's building.  If your physicial is dated 30 days prior to the start of the season you MUST complete a new permission slip AND yellow update card.
