Sports Clearance & Final Forms

Every athlete must register with Final Forms. 

After registering, you must first add the sport your child is trying out for

After choosing a sport, you must complete the rest of the paperwork (ie. parent signature, student signature, update form).  All of these things must be completed in order for the school doctor to be able to review/clear your child for sports.  Please note: if you do the signatures and update form before adding a sport, adding the sport will reset the signatures and update so it's important to do these things in the correct order outlined below:

1.  Add Sport

2. Complete all forms

3.  Once everything else is in green, the doctor will be able to review/clear your child for sports.

 Email notifications will be sent upon clearance.

Registration for JV & Varsity winter sports 2019 opens on Final Forms 10/14

 (new private physicals due to health office 10/28)

First day of JV & Varsity winter season is TUESDAY 11/12

(new private physicals due to health office 1/16)

First day of JHS late winter sports is TUESDAY 1/21

SUFFOLK SPORTS HALL OF FAME ANNOUNCES CLASS OF 2019

Suffolk Sports Hall of Fame Class of 2019 honorees ... 

Ward Melville Boys Lacrosse: When you think of boys’ lacrosse, you think of Ward Melville, another one of this year’s Special Recognition Award honorees. The perennial contender is the dynasty of dynasties with 10 New York State titles to its credit, including most recently in 2018 when they finished ranked as the No. 1 school in the country.
