Sports Physical Information
Spring 2012 Sports Physical Dates:
WMHS - 2/28/12 @ 2:00 p.m.
Gelinas - 2/29/12 @ 2:30 p.m.
Murphy - 3/1/12 @ 2:30 p.m.
All athletes must be screened by their school nurse prior to the physical date. The 3 forms necessary are located to the left "Sports Physical Forms".
REMINDER
For participation on a spring sport 2011/12
all Private Physician Physicals -
JV & Var (Grades 9-12) DUE BY 2/27/12
7/8th grade DUE BY 3/12/12
Return all forms to the health office in your child's building. If your physicial is dated 30 days prior to the start of the season you MUST complete a new permission slip AND yellow update card.
If you have an approved physical on file with the nurses office for the fall athletic season you will need to complete a permission slip & yellow update card to participate in the winter season. Permission slips & yellow update cards can be picked up in the health office.
School Provided Sports Physicals
All athletes must be screened by their school nurse prior to being seen by the school physician. Athletes need to be dressed in appropriate attire (shorts & t-shirt) to be seen by the physician. The athletic physical forms can be downloaded from the links on the left or under the "Important Forms" section.
Private Physician Sports Physicals
Athletes may receive their sports physicals from their family physician. Three Village CSD Athletic Physical forms (3 forms on the left) must be completed by your private physician.
All private physician physicals must be cleared by the district physician prior to participation.
FOR PARTICIPATION IN A SPRING SPORT 2011/2012 SEASON ALL PRIVATE PHYSICIAN PHYSICALS MUST BE HANDED IN TO THE NURSES OFFICE AT YOUR CHILD'S SCHOOL NO LATER -SEE BELOW. ALL PRIVATE PHYSICIAN PHYSICALS MUST BE CLEARED BY THE DISTRICT PHYSICIAN PRIOR TO PARTICIPATION. ALL PRIVATE PHYSICIAN PHYSICALS MUST INCLUDE:
- HEARING - WITH NUMERICAL VALUES
- VISION - WITH NUMERICAL VALUES
- URINALYSIS
