Gelinas - 8/25 7:30 a.m.
All athletes must be screened by their school nurse prior to being seen by the school physician. Athletes need to be dressed in appropriate attire (shorts & t-shirt) to be seen by the physician. The athletic physical forms can be downloaded from the links below or under the "Important Forms" section.
~NEW FOR THE 2010-2011 SCHOOL YEAR~
Athletes may receive their sports physicals from their family physician, this physical MUST include a complete hearing, vision and urinalysis. Three Village CSD Athletic Physical forms (3 forms listed above) must be completed by your family physician and brought to the nurse in the athlete's school for review. Once cleared by the nurse, all forms will be sent to the district physician for final clearance. The deadline to have physicals from your family physician in for approval are as follows:
All Varsity/JV Football Players no later than 8/6
All other athletes no later than 8/20
